MEMBERSHIP IS REQUIRED TO REGISTER AND PRACTICE AT THE EISD FACILITY.
IF YOU ARE CHOOSING THE FREE TRIAL, COMPLETE THE REGISTRATION PAGE AND SIGN THE WAIVER TO PARTICIPATE IN ONE PRACTICE.
